Has your cat been around children?

I understand that any cat(s)I foster is/are the property of NTCR, and I agree to turn the cat(s) over to NTCR immediately upon request. I agree to bring any cat(s) fostered by me to the Veterinarians NTCR works with for exams, vaccinations, weight checks, sterilization, or other reason deemed necessary by NTCR and mutually agreed upon date and/or time.

I understand that I am responsible for adhering to all NTCR standards of care. A pamphlet " How to care for a cat for North Texas Cat Rescue " will be available to each foster.

Any cat(s) fostered by me will be fed, watered and exercised appropriately. I agree to provide an adequate and nutritious diet, including any necessary supplements, as recommended by NTCR. I agree to abide by any instructions given by NTCR regarding the feeding of specific brands or types of supplemental food and/or nutritional supplements to my foster cat(s). Although NTCR will make every attempt to furnish food and/or litter for those foster care volunteers who are unable to afford these supplies on their own, I understand that the availability of these supplies is based on donations from the public and limited to stock on hand and that I will be responsible for providing necessary supplies in the event that they are not available through NTCR. NTCR cannot and does not guarantee to be able to provide free food, litter or supplies for my foster cats on a consistent basis. NTCR partners with low cost suppliers to acquire discounted food/litter foster volunteers can then purchase.

I will not allow fostered cats or kittens to go outside at ANY time except to transport them to and from the foster home or veterinarian, if needed. I agree to transport any foster cats, kittens in a sturdy carrier. I agree to transport any foster cat(s) in an enclosed vehicle only, and I will not allow them to ride in the bed of a pickup truck or convertible.

I understand that NTCR will take every precaution to ensure that any cat(s) I foster are reasonably healthy and that any known health problems will be disclosed to me prior to my taking the animal into my home, but NTCR cannot be responsible for any unforeseen health problem that may develop after the animal(s) is/are in my care. If all possible NTCR will test all cats 8 weeks and older for FIV and FelV. NTCR will supply wormer, flea treatment, vaccines and any medicines to treat diarrhea, URI and ringworm. I AGREE TO KEEP the new cat(s) IN ISOLATION FOR 2 WEEKS (14 DAYS) COMPLETELY SEPARATED FROM MY PERSONAL CATS OR OTHER FOSTERED CATS.

I understand that I am not authorized to seek outside veterinary care for NTCR foster cat(s)/kitten(s) without approval from NTCR Board Members or the NTCR foster Coordination team and may be dismissed from the foster program for doing so. If I choose to treat the cat(s) at a veterinarian of my choice, I understand that I will assume full responsibility for payment of the vet bill, and that NTCR will be unable to reimburse me for any such payment unless pre-arranged.

I agree not to foster cats from any other organization while I am fostering for NTCR, UNLESS GRANTED SPECIFIC APPROVAL BY NTCR Board of Directors. I agree not to bring in my house cats coming from the streets, shelters or friends WITHOUT seeking authorization from NTCR Board.

I understand that Foster Care Coordinator/ NTCR Board Members have the right to limit the number of animals in my care at any time

I understand that NTCR cannot and does not make any representations or warranties, either expressed or implied concerning the temperament, habits, health, pedigree, disposition, age, sex or background of this animal and that I have no right to a reimbursement of medical, adoption or other professional fees. Furthermore, I understand the animal's future reactions to me, my family or any other person is completely unpredictable because animals, like people have their own personalities. Should the animal I am fostering exhibits anti-social behaviors NTCR will counsel, offer recommendations yet if all fails NTCR will make arrangement to remove the said animal from my house to reoacate in a safe environment.

If for any reason, I do not comply with all provisions of this agreement, I agree to return the fostered cat(s) to NTCR upon demand. If I refuse to return the fostered cat(s) to NTCR upon demand, I understand that NTCR reserves the right to remove the cat(s) I foster from my home

If I decide to adopt my fostered cat/kitten, I understand I will receive a 25% discount.

I agree to enter all required treatments like deworming, flea treatment, ringworm treatment or any other treatment recommended by NTCR in the database I have been granted access to. I understand it is my responsibility to keep each cat's medical journal updated. NTCR counselors will vaccinate the cats based on a schedule. It is important I make sure my fostered cat(s) is/are boosted as required. Should a cat I foster bites/scratches a member of my family or a visitor I will be responsible for the expenses incurred to quarantine the cat at a veterinary clinic as per Animal Control in case the cat is not up to date on Rabies vaccine. It is my responsibility to contact NTCR to schedule a Rabies vaccine if the cat is 12 weeks old. NTCR schedules all appointments at the Veterinary Clinic .

Are you willing to bring the foster cat/kitten to the adoption center at Petsmart in Allen on adoption days? Adoption events are held on Saturdays and/or Sundays from 1 PM to 4 PM

In condition of receiving any NTCR foster cat(s), I as a foster caregiver, hereby release NTCR , its officers from all claims of injury or damage that a fostered cat may cause me or my property. I agree to hold harmless, defend and indemnify NTCR, its officers, veterinarians from any and all claims of liability to other persons for injuries or damage arising out of or in connection with services of this program or cause to them or their property by the cat(s) I am fostering through NTCR
